In order to explore this further, it is helpful to refer to the NICE and the RCPCH guidance issued to explain when it may be appropriate to suspect FII.

The NICE Guidelines state the following. Those indicators which could present a potential risk to parents of children with ‘perplexing presentations’ are highlighted in italics.

Clinicians should suspect that illness may be fabricated or induced if the history, the physical or psychological presentation of the child and any findings of investigations or assessments do not match a recognised clinical picture and, in addition, one or more of the features below are present:

Any of the symptoms noted are only observed in the presence of the parent or carer.

Any issues or problems are only seen by the parent or carer.

The child demonstrates a poor response to any medication that is prescribed.

The parent or carer begins to report new symptoms once the previously reported symptoms are resolved.

The parent or carer reports biologically unlikely symptoms (for example they may reports significant blood loss but the child is not visibly unwell or anaemic).

Despite having undergone assessment, and a definite clinical outcome having been agreed, the parent/carer continues to dispute the outcome and seeks further investigations.
